- Click on the Care for the Elderly Section
- Select the Service that you would like to apply for (Note: Services which are marked with the note “Medical Report Required” indicate that in order to apply, a Medical Report of this application must be completed by your family doctor and endorsed with an official stamp and his/her signature respectively.)
- Fill in the additional Details
- Click the proceed button
- Fill in the Applicant details
- Fill in the Next of Kin details
- Confirm that the information given in is true, complete and accurate
- Click on the Submit button
